**Teen to Adult Transition-Specific Program**
The "Ready to Fly" program is designed to facilitate the transition of adolescents to adulthood through a comprehensive array of services, which includes:
- Life skills training encompassing self-care, cooking, money management, and community mobility.
- Employment preparation and vocational services provided through Vocational Rehabilitation. This includes employment-related activities and resources.
- Development of structured independent living plans, which focus on goal setting, budgeting, and housing skills.
- Support mechanisms that are consistent with the transition planning outlined in the Individuals with Disabilities Education Act (IDEA) and the Oregon Youth Transition Services.
This program operates within a residential setting that offers 24-hour support for cohorts of 1 to 5 adolescents in a homelike environment.
- All staff members undergo thorough background checks.
- A registered nurse is available on-site to oversee medical care and medication administration records for each client.
Eligibility and intake assessments will be conducted for each participant in the Ready to Fly program.
Participants must:
- Be deemed eligible for developmental disability services through the Community Developmental Disabilities Program (CDDP) and the Office of Developmental Disabilities Services (ODDS).
- Meet the eligibility criteria for school-aged youth with developmental disabilities, which extends up to the age of 22.
The Ready to Fly program serves youth aged 14 to 22, in alignment with the initiation of transition planning required by IDEA, which is mandated to begin by age 16; however, some individuals may start at age 14. An application process and interview are prerequisites for participation in the program.
The Ready to Fly program also offers Service Planning and Support, which includes:
- implementation of an Individual Support Plan (ISP) for each participant. This plan encompasses transition services, health and safety measures, adjustment supports, and consultations.
- Coordination of planning with:
- Family members and caregivers
- Case managers affiliated with CDDP or brokerages
- Transition facilitators and Vocational Rehabilitation (VR) representatives
- Educational professionals to harmonize school-to-independent living goals.
The ISP commences a twelve-month initial period post-admission.
For clients with significant behavioral needs, a Behavioral Support Specialist is available to create tailored plans and monitor progress.
**Staffing and Clinical Oversight**
Staff members must satisfy regulatory requirements, including background checks, training, and certification in CPR/First Aid.

The Ready to Fly program is located in a rural residence in Veneta, Oregon.
This facility provides room and board within a supportive, home-like environment. The program emphasizes the development of life skills and facilitates the transition from adolescence to adulthood, while ensuring ongoing support in both home and community settings.
